The useful starting point
A project note may mix the reason for a change, the agreed decision and work still to do. Those statements have different roles. Without clear separation, a proposed idea can be mistaken for something already agreed.
A small step to try
Use three small sections when the distinction matters. In the decision section, identify what was actually agreed and by whom where appropriate. In the next step, name the work and owner. Keep unresolved questions visibly open. The structure is a practical drafting option, not a mandatory format; choose it when it makes the status of the information easier to understand.
